This form is designed to capture the details of any incidents (e.g. collisions, trips etc.) that occur and are related to the new Cycle Enfield infrastructure.
All fields must be completed for the form to be accepted. Please provide as much detail as possible so that we can understand the incident and where exactly it occurred.
All incidents raised will be logged and will go through a process of review. Where necessary the Healthy Streets team will follow up with the person directly involved.
This information is used to identify specific locations in the Borough where accidents are occurring more frequently, and conduct monitoring and changes if necessary.
